FHSAA Paperwork

ALL STUDENT-ATHLETES MUST HAVE A COMPLETED EL2 FORM ON FILE IN ORDER TO PARTICIPATE IN ANY JV OR VARSITY PRACTICE OR GAME. This is the only Physical form the FHSAA will accept.

• FHSAA EL2 Physical Form

ECG Screening form - required form starting July 1, 2026 for all incoming 9th graders and new student athletes in grades 10-12.

•  FHSAA EL1 Electrocardiogram (ECG) Screening

The form below is only for student-athletes that have transferred to Springstead High School from another High School and only has to be completed once. This form must be completed and on file, prior to participation in a sport at Springstead.
